The Life Plan Implementation Package provides four weeks of individualized support to help families and young adults take meaningful steps toward their goals.
What's included:
One 60-minute initial planning session
Two 45-minute follow-up sessions
Assistance with agreed-upon applications, resource navigation, or agency communication
A personalized action checklist
One final progress summary with recommended next steps
Areas of support may include:
Postsecondary education and training
Employment and career preparation
Independent living planning
Community resources and support services
Self-advocacy and personal organization
Investment: $400
Booking: A $200 deposit is required to begin. The remaining $200 is due before the second week of services.
The package includes up to three scheduled sessions and agreed-upon support activities during the four-week period. Additional services are quoted separately.
